Patientenselbstbestimmung und Patientenverfügungen haben zunehmende Bedeutung und Beachtung erfahren. In der vorliegenden qualitativen Studie wurden 15 Patientinnen und Patienten mit amyotropher Lateralsklerose —einer unheilbaren, chronisch-degenerativen Erkrankung mit vorhersehbarer Symptomatik—interviewt, um zu erfahren, welche Werte und Kriterien sie bei prospektiven Entscheidungen am Lebensende und bei der Abfassung von PV zugrunde legen. Die Auswertung erfolgte nach der Methode der „grounded theory“. Die befragten Patientinnen und Patienten befürworteten einen Verzicht auf lebenserhaltende Behandlungen, wenn sie keine hinreichenden Lebensmöglichkeiten mehr sahen, d. h. wenn sie (...) die aktuelle Lebensqualität für unzureichend erachteten und die Chance auf Besserung als gering einschätzten. Dem Patientenwillen sprachen sie bei der Entscheidungsfindung ein Vetorecht zu. Patientenverfügungen fassten sie erst ab, als sie keine Hoffnung mehr auf Besserung ihrer Erkrankung hatten. Sie verstanden diese als Instrumente zur vorsorglichen Erklärung ihres Wunsches, auf lebensverlängernde Maßnahmen zu verzichten, wenn die LM inakzeptabel geworden sind. Damit wollten sie sich gegenüber dem Arzt absichern, den sie in der Rolle eines Anwalts des Lebens sahen und mit dem sie daher ihre Behandlungswünsche nicht besprochen haben. Trotz vorhersehbarer Symptomatik legten sie in ihren PV keine spezifischen Behandlungswünsche nieder, sondern verwendeten allgemeine Formularmuster ohne konkreten Bezug zu ihrer Erkrankung. Daher bestehen auch bei dieser Patientengruppe Zweifel, ob die PV in einer konkreten Behandlungssituation hilfreich sind. (shrink)

Originally an annual journal, its regularity has been disturbed due to financial difficulties. The study acknowledges that though mining has contributed to the development of many economies globally, it comes with unanticipated challenges especially on the people who are directly affected by the mining activities. The study aims to understand the implications of such challenges faced in mining resettled communities and the versatile practices followed to stay in the mist of such livelihood challenges in mining resettlement communities. The study also focuses on the copying strategies adapted to cope with these challenges in mining (...) resettlement communities. Qualitative approach was done to capture the rich essence of the subject matter. The study used focus groups and in-depth interviews as techniques and interviewed members from different households of the mining resettlements communities (Salman, Teleku-Bokazo and Nkroful) including land owners, opinion leaders, chiefs and from the institutional perspectives. As a result, it was found out that the livelihood challenges that clearly manifested itself was in the area of income, employment and morbidity issues. The evidence was shown that in all the challenges, copying measures are in place to enable the people live in their new settlements, but not adequate. (shrink)

The Tattvabindu of Vacaspatimisra with the commentary called Tattvavibhavand of Paramesvara II of Payyur Bhattamana. This edition of Vacaspatimisra's Tattvabindu and of its commentary Tattvavibhavana by Paramesvara II is based on (1) a transcript of a manuscript Tattvavibhavana preserved in the Madras Government Oriental Manuscripts Library, and (2) the Benares Edition of the Tattvabindu. Since the commentator has made it a rule to quote the full text by parts before commenting on it. Vacaspatimisra's Tattvabindu is a short and highly difficult (...) text. In his characteristically rhythmic and stately diction, Vacaspatimisra reviews in the Tattvabindu, the vaiyakarana doctrine of Sphota, mainly as expounded by Bhartrhari and Mandanamisra, and amplifies and reaffirms Kumarila's criticism of Sphota doctrine. The elaborate and scholarly introduction, giving a historical sketch of the Mimamsa literature, and the thoroughgoing analysis of the Tattvabindu, in English, which the learned editor has prefixed to his edition of the work. In the last century and in the first decades of the present century the historians of India laid stress on the isolation of the subcontinent by the mountains and seas surrounding her on all sides and cutting her off as a separate universe. The progress of modern research has shown how mistaken this view was. We now see the true facts much more clearly than ever. The mountain barriers, though formidable at many points, are broken by gaps which have always (...) allowed a considerable intercourse across the frontiers on the northwest as well as the northeast and at some points due north as well. By her position in the center of the littoral of the Indian Ocean, India enjoyed ample facilities for communication by sea with the countries lying to her west and east, and we now know that she and her neighbors availed themselves of these facilities from time immemorial. The Indian Ocean was navigated freely from very early, even prehistoric, times, and there was a series of lively maritime exchanges—migratory, commercial, and cultural—among the peoples of Africa, India, and Indonesia, or more generally Southeast Asia. Western Asia and the lands adjoining the Levant also took, from the beginning, a prominent part in this commerce of goods and ideas between India and the world. It is the aim of this paper to trace briefly the ancient contacts between India and the Western countries to the time when the rise and spread of Islam and the Islamic impact on India ushered in a new epoch. Semantic relationships have an active role in textual study, as they are an essential tool through which the text is constructed, this is done through the sequence of sentences of saying, and these relationships lead to the growth and continuation of the subject of the text, and then linking speech, which in turn achieves textual harmony. The aim of this study is to highlight the importance of these relationships and to elaborate on them, this research dealt with two additional linkage (...) relationships: (equivalent additional relationship), and (different additional relationship), with a breakdown of each of them, and then the inclusion of poetic applications from the texts of the poet Hazem Rushak Al-Tamimi, showing the extent of the harmony, plot and coherence of these texts. This study investigates the intentions, perceptions, and actual implementation of an innovative English curriculum among preparatory school EFL teachers in Iraq. The research recognizes the growing importance of English globally and Iraq's efforts to enhance English language education and implement an appropriate innovative curriculum. However, the implementation of a new curriculum poses significant challenges related to teachers' knowledge, intention, and academic culture. The study focuses on understanding the relationship between curriculum intentions, teachers' perceptions, and the actual implementation of the curriculum. (...) The study also explores how teachers' intention, culture, and other factors influence the implementation of the curriculum. Data were collected through a combination of vignettes, semi-structured interviews, classroom observations, and follow-up interviews with a random sample of 10 male teachers from 10 preparatory schools in Babylon Governorate during the 2023-2024 school years. The results indicated that teachers are enthusiastic about the new curriculum, but its effective implementation faces many challenges. Teachers recognize the potential of the curriculum, but suffer from insufficient materials and overcrowded classrooms that hinder optimal teaching and learning. The study emphasizes that in order to successfully implement the curriculum, through continuous professional development and strong support systems, there must be ongoing professional development and strong support systems, ongoing training programs can help teachers adapt to the new curriculum, and the provision of resources, teaching aids, and manageable class size can create a more conducive learning environment, leading to better learning outcomes. The folk tale is one of the means that preserve cultural heritage and consolidate cultural identity. It is one of the most important forms of expression. It is a summary of the experiences of generations, formulated in an interesting narrative form, full of lessons and noble values. It is a creation of the imagination in which the wisdom of society is revealed. It is a reflective mirror of the society in which it grew, so that the story is considered Folklore is one of the genres of popular literature that is beloved by adults and children, due to the elements it contains that attract, excite, and enrich the imagination, in addition to the positive values it contains. It is the ancient, common art of spontaneous speech, passed down from generation to generation, linked to customs and traditions. The story is the backbone of popular heritage, and it is what we metaphorically call popular literature. Among the studies of the folktale in which the researcher invested the importance of the folktale is a study by M. Entscheidungen der Therapiebegrenzung und in der Betreuung am Lebensende sind häufig komplex und von ethischen Problemen begleitet. Im Mittelpunkt der Untersuchung steht die entscheidende Frage, wie hilfreich existierende „Ethik-Richtlinien“, die eine ethische Orientierung bei solchen Entscheidungen geben sollen, in der klinischen Praxis tatsächlich sind. Die Frage, welchen Nutzen „Ethik-Richtlinien“ bei der Entscheidungsfindung haben oder haben können, wird hier exemplarisch an einem klinischen Fallbeispiel aus einer Ethik-Kooperationsstudie in der Intensivmedizin analysiert. Vergleichend werden hierzu „Ethik-Richtlinien“ aus Deutschland, der Schweiz und aus Großbritannien (...) herangezogen, die Gegenstand eines internationalen Projekts zur Analyse von Richtlinien waren. Die Möglichkeiten und Grenzen einer ethischen Orientierung an „Ethik-Richtlinien“ bei Entscheidungsproblemen der Therapiebegrenzung und in der Betreuung am Lebensende werden anhand der Fallstudie diskutiert und illustriert. Abschließend werden Schlussfolgerungen für die Entwicklung ethischer Richtlinien für die klinische Praxis formuliert. (shrink)
